For referral request to a specialist, please call our office. You may have to see our provider before a referral can be provided. We request 3-day notice to process a referral. An urgent referral request with less than 24 hours notice may incur an fee of $25.

Based on your insurance plan, an authorization may be needed for the referral and it is your responsibility to find out from your insurance company. You will also need to verify that desired specialist is “in network” in your health insurance plan.

Referral Request Steps

1. Check with your insurance company if the specialist is a participating physician in your plan.
2. Make appointment with the specialist and verify their office if still a participating provider with your insurance. Perform this step even if specialist’s appointment needs to be expedited by our office.
3. Verify if the in-network specialist requires an authorization form.
4. If your insurer requires any special form, provide our office with the following information:
   • Specialist name, phone number and your appointment date/ time
5. Prior to your appointment, verify if specialist has received the authorization/ referral form.
